Card Renewals 2008/2009
The OSAC Steering Committee is now accepting applications for the renewal of OSAC Cards for 2008/2009. The limit for OSAC Cards will be 5 per company/organization. 1 Executive Card is permitted out of these 5 cards.
Requirements (all applicants must be employees of a US Company/Organization):
Cost (all in US Dollars)
Executive Card: USD 150: Note that this card will only be issued to one company CEO/GM/Director.
Each individual OSAC Card will cost USD 100
OSAC Cards are valid for one year. Please indicate on application which checkpoints you wish to gain access to.
Please contact Mahmoud Abu Radaha from the Financial Committee (Contact information listed below) for OSAC Bank Account number and method of payment for OSAC Cards.
If the applicant is... he/she must provide:
1. Foreign passport holder - he/she must provide a photocopy of a valid visa into Israel and attach it to the application.
2. Jerusalem/Blue ID Holder - he/she must provide a photocopy of a police report which they may get from any Israeli police station. (They need to first go to the post office and get stamps for the police report. Then they go to the police office and give them the ID number and the stamps. The police will then print a report out for them on the spot).
3. Palestinian ID holders only need to provide a photocopy of their ID and a valid magnetic card issued by the Israeli DCL.
Note: USAID badge holders are not permitted to apply for OSAC Cards whilst carrying a USAID badge. No "dual card holding" is permitted. OSAC membership will not be affected.
Please provide me with a completed application sheet and send to General Motors Company (Kirresh Auto Company) in either Ramallah or Al Ram locations.
